Substitute Notice of Potential HIPAA Breach

This notice is provided by Central Health Medicare Plan concerning a potential HIPAA breach that may affect the personal health information of members. On February 22, 2017, Central Health Medicare Plan discovered that a package containing approximately 100 paper claims was missing. This package was prepared to be sent out via FedEx on January 4, 2017, but was not delivered and has not been located. The packaged paper claims may or may not contain the following personal health information of members:

  • Name
  • Central Health Medicare Plan Member ID
  • Social Security Number
  • Address
  • Date of Birth
  • Diagnosis Codes/Medical Procedures
  • Provider Information

Central Health Medicare Plan has been thoroughly investigating the matter and is working to locate the missing package. We have taken steps to remediate the issue to prevent future incidents from reoccurring.

As a precaution, we recommend Central Health Medicare Plan members take the following steps to protect against possible identity theft or other financial loss:

  • Register a fraud alert and order credit reports from the following three credit bureaus:
      • Experian: (888) 397-3742; www.experian.com; P.O. Box 4500, Allen, TX 75013
      • TransUnion: (800) 680-7289; www.transunion.com; P.O. Box 2000, Chester, PA 19022
      • Equifax: (800) 525-6285; www.equifax.com; Fraud Assistance, P.O. Box 740241, Atlanta, GA 30374-0241
    • U.S. residents are entitled under U.S. law to one free credit report annually from these three credit bureaus
  • Monitor account statements, explanation of benefits (EOBs), and credit bureau reports closely.
