How To Enroll 

If you are interested in joining Central Health Medicare Plan, you have several ways to do so:

1. Call Central Health Medicare Plan at 1-866-314-2427 (TTY/TTD: 1-888-205-7671) to obtain an enrollment packet or have a Central Health Medicare plan representative assist you in filling out an enrollment application over the phone.

2. Attend a Central Health Medicare Plan Informational Sales Meeting.

3. Download the Central Health Medicare Plan Membership Application Form and mail it to the address below:
