If you are interested in joining Central Health Medicare Plan, you have several
ways to do so:
Call Central Health Medicare Plan at 1-866-314-2427 (TTY/TTD: 1-888-205-7671) to obtain an enrollment packet or have a Central Health Medicare plan representative assist you in filling out an enrollment application over the
phone. We are available 7 days a week from 8:00am to 8:00pm (PST).
Attend a Central Health Medicare Plan Informational Sales Meeting.

Central Health Medicare Plan is an HMO with a Medicare contract. Central Health Medicare Plan’s contract with CMS is renewed annually and the availability of coverage beyond the end of the current contract year is not guaranteed.
You must have both Medicare Part A and Part B, and reside in the service area of the plan to enroll. If enrolling in our Special Needs Plan, you must also receive assistance from Medi-Cal. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. You cannot enroll in this plan if your current or former employer helps pay for your drugs.
You must use plan providers except in emergent or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-plan providers neither Medicare nor Central Health Medicare Plan will be responsible for the costs.
The Medicare Prescription Drug Benefit is only available to members enrolled in Central Health Medicare Plan, a Medicare Advantage with Part D (MA-PD) plan.
You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for getting extra help, you can call 1-800-MEDICARE (1-800-633-4227). TYY/TDD users should call 1-877-486-2048, 24 hours a day/7 days a week. You can also call your State Medicaid Office or the Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY/TDD users should call, 1-800-325-0778.
Your ability to enroll may be limited to certain times of the year. Medicare beneficiaries may enroll in Central Health Medicare Plan through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.medicare.gov.
