Enrollment Eligibility Requirements
To be eligible to enroll in Central Health Medicare Plan, a person must:
Be enrolled in Part A and Part B of Medicare
A Medicare beneficiary is generally eligible to enroll in Central Health Medicare
Plan if he or she is entitled to Medicare Part A and enrolled in Medicare Part B
as of the effective date of enrollment in Central Health Medicare Plan. If you are
entitled to Social Security benefits either on your own work record or your spouse's
record there is no charge for Part A. If you would like to purchase Part A from
Social Security, please call your local social security office or call 1-800-772-1213.
For the hearing-impaired, the toll free number for Social Security is 1-800-325-0778.
Not be diagnosed with end stage renal disease (ESRD)
If you have ESRD, you cannot enroll in Central Health Medicare Plan. If you no longer
need regular dialysis, or have had a successful kidney transplant, you may submit
a note or records from your doctor confirming this.
Be a resident of the Central Health Medicare Plan service area (Los Angeles
County)
Complete the Enrollment Form
A Medicare beneficiary must complete, sign, and date the Central Health Medicare
Plan to enroll in one of our plans.
