Central Health Medicare Plan has two plan offerings for 2011.
Central Health Medicare Plan is a Health Maintenance Organization (HMO)
that also includes prescription drug coverage. This gives you the convenience of having
all your needs – medical and drug – coordinated by one plan.
Central Health Medicare Plan also gives you access to our extensive group of medical
providers along with the flexibility to choose among them. You can choose a Primary Care Provider
from a large group of highly skilled physicians in your area. You can expect excellent care.
Plus, with our HMO, there's virtually no paperwork.
Central Health Medicare Plan gives you complete access to our Prescription Drug
Plan. Our drug plan is designed specifically for Medicare beneficiaries, and includes
many of the most commonly prescribed brand name drugs for the conditions most likely
to affect you, at an affordable price.
Central Health Medi-Medi Plan (Special Needs Plan) is our Medicare/Medicaid
Dual Eligibility Plan and is only available to those who have qualified financial
status, as determined by government guidelines. Central Health Medi-Medi Plan is
a Special Needs Plan and includes access to our Prescription Drug Plan. This plan
provides reduced or eliminated premiums and reduced coinsurance or copayment to
those who need it most.
Eligibility for Central Health Medi-Medi Plan is determined by the state, so if
you've received a qualifying letter from the government and need to talk to someone
about enrolling in Central Health Medi-Medi Plan, please call us at 1-866-314-2427

8:00pm (PST).

How Can I Get Extra Help With My Prescription Drug Plan Costs or Get Extra Help With Other Medicare Costs?
You may be able to get extra help to pay for your prescription drug premiums and costs as well as get help with other Medicare costs. To see if you qualify for getting extra help, call:
